PLAN TERMS:

• The Cardinal Care Club is NOT insurance. It is a savings plan for patients without dental insurance.

• It can not be used in conjunction with any other plan, insurance, coupon, or discount.

• There is no refund of the premium if the participant decided not to utilize the benefits. Unused benefits will not be carried over to the following year.

• This plan can only be used at Cardinal Dental and Fountain Valley Smiles.

• Discounts are taken off of our full price (UCR) fees.
